The Gift of Wellness consists of an annual check-up package to be availed of at any of the Healthway Medical Clinics, which includes the following:

 

• ​Routine Urinalysis

• Routine Fecalysis

• Digitized Chest X-Ray

• Complete Blood Count

• Complete Medical Exam

      (Oral Exam, Visual Acuity, Medical History)

• Post evaluation result of APE with the Primary Care Physician

 

Who may avail of the gift?

NEW policyholders and EXISTING policyholders* with renewed policies.

 

*Only the policyholder of the following line may avail this gift:

Motor, Fire, Viajes, Premier PA, HCPP, Contractors and Suppliers Bond.

*Subject to a minimum premium and payment of premium requirement.

 

Steps for Insured to avail of the Gift of Wellness:

 

• Call our Teleservice  (632-8764400) or email teleservice@mapfreinsular.com and ask for the Gift of Wellness.  

• Please be ready to give your policy number and personal information details (name, policy number, contact number, email address/mailing address) and indicate your preferred mode of delivery of your certificate (fax, mail or email).

• Present the certificate to any of the **Healthway Medical Clinics.  It is advisable to call the clinic for an appointment prior to your check-up.

• The results of the check-up will be available for pick-up at the clinic one week after the exam.
